肝臟疾病(肝炎、肝硬化、肝腫瘤、肝癌)是華人(國人)常見的疾病,早期肝臟的診斷與治療是避免惡化很重要的措施。肝臟的診斷除了生化檢驗外(GOT/GPT, α球蛋白),超音波常被用來做第一線的篩選診斷,進一步的確認則常使用電腦斷層影像(CT)。若病症需要治療,則使用CT導引穿刺取樣或酒精注射或RF燒灼或動脈血管栓塞,嚴重者則進行(微創)肝臟切除手術、標靶藥物治療。超音波影像掃瞄與判讀,其學習週期是很長的,通常靠臨床經驗提升判讀的正確性。為了降低誤判,本計畫擬以立體定位技術偵測超音波探頭掃瞄時的方位,以建立具方位的超音波影像資料庫。之後應用影像處理與分割與數學運算,讓超音波影像與所建構的3D電腦斷層影像模型相互對應(mapping),達到臨床上以電腦斷層影像輔助超音波影像的判讀。此一技術將應用在肝臟治療上,協助醫師在超音波影像下進行穿刺取樣或酒精注射或RF燒灼所需的定位。 Liver disease (Hepatitis, Cirrhosis, Tumor, Cancer) is a very common disease of Chinese. Early diagnosis and treatment of liver disease is very important measures to avoid worsening. Except biochemical test such as GOT/GOP or α-globulin), ultrasound scanning is usually adopted for first-line screening and diagnosis. Computed tomography is scanned for further confirmation. If the disease needs treatment, tissue biopsy, alcohol injection, RF burning, or artery embolism will be done under CT guidance. For serious cases, minimally invasive liver dissection or target medicine treatment will be necessary. The learning period of ultrasound scanning and diagnosis is very long. The accuracy of interpretation usually relies on clinical experience. To minimize incorrect diagnosis, this research will apply stereotactic positioning technology to detect the position and orientation of ultrasound probe during scanning and to form a database of ultrasound images. After that, the mapping of ultrasound images onto 3D CT model can be done using image processing and segmentation and mathematics, which can be thought as the combination of CT images and US images for diagnosis. Such a technique will be applied to liver treatment to assist surgeons to proceed tool positioning for liver biopsy, alcohol injection and RF burning. 研究期間:9908 ~ 10007
