    Title: 以互動式虛擬實境上肢復健系統探討生物心理模式與中風復健成效關聯性、並建立以醫學實證為基礎之臨床評估方法;Study Correlations between Bio-Psych Social Model and Effectiveness of Post-Stroke Rehabilitation and Develop Clinical Assessment Methods via an Interactive Vr Upper-Limb Rehabilitation System

    Abstract: 根據臨床資料顯示,88%的急性中風病患呈現上肢偏癱症狀,需經由復健提昇生活品質。傳統物理治療或是職能治療的訓練手法,缺乏階層式的運動復健模式以針對個別病患進行彈性的復健策略規劃或療程設計;同時,現行的臨床復健成效評估,根據有限的資訊型態,不易快速而精準地反應病人複雜的生理狀態;同時,病人於復健過程中的心理情緒因素,也是容易被忽略的。本計畫擬針對腦中風上肢復健運動,結合體感互動、觸覺模擬、遊戲動畫以及3D視覺顯示等技術,建立指腹夾取運動、手腕運動、手軸運動以及肩肘運動之互動式虛擬實境復健任務系統;設計以物理為基礎之視覺、聽覺以及觸覺的即時感知反饋以及動作任務引導,運用物理參數的各式組合,建立系統化且階層式的運動復健模式;設計相關復健策略及療程,進行中風病人臨床試驗,量測多維度生理表現資訊,包括臨床復健成效評估、運動分析、任務表現以及腦電波量測,整合上述資訊資訊,發展以醫學實證為基礎之新型評估方法;同時也量測復健過程中的多維度心理資訊,包括科技接受度、情緒、存在感、悅趣度以及使用意向,建立生物心理模式,瞭解多維度心理資訊與多維度生理表現資訊之相關性,探討生物心理社會模式對於復健表現與成效的影響。In Taiwan, more and more people annually suffer a stroke and this has been cited as one of the leading causes of severe, long-term disability. According to clinical reports, 88% post-stroke patients are with upper-limb impairments that the living quality is highly impacted. Recent advances in the understanding of neural plasticity and motor function retraining clearly suggest that greater quantity, duration and intensity of practice are most effective in modifying neural reorganization. Substantial effort and expertise has been focused on developing innovative technology/interventions that exploit the neuroplastic properties associated with the sensorimotor systems in the adult brain. As a result, motor rehabilitation is needed in order to recover required motor functionality and improve the living quality. However, traditional methods used by physical therapists are insufficient in providing a hierarchical system, therefore physical therapists are not able to make a specific therapy planning and protocol design to each patient. Besides, forms of motor information collected in clinics are limited that patient’s current status is not easy to be revealed accurately and precisely. Further, a lot psychology issues are generally neglected while patient’s motivation to continue the participation of therapy plays an important role in getting rehabilitation succeeded. In this project, we propose to develop an interactive VR-based tasks-oriented motor rehabilitation system aiming at several upper-limb motor exercises which includes palm pinch, wrist flexion/extension/deviation, elbow pronation/supination and shoulder-elbow movement. This system utilizes various advantages of innovative technologies such as interactive technologies, haptics simulation, 3D games and stereoscopics, therefore to provide various forms of instant physics-based sensory feedbacks (visual, acoustic and force) and guides to accomplish tasks. Also, this system allows therapists to design a hierarchical training plan with proper quantity, duration and intensity of practice via the combination of various physics-based parameters. To verify the effectiveness of the system, we plan to conduct a large scale clinical experiment. Multi-dimensional performance or physiological data, including clinical assessment results, task performances, motor information and electroencephalography (EEG), are measured in order to develop integrated clinical assessment methods that can accurately and precisely reveal patient’s current status and evaluate the progress. Beside, multi-dimensional psychological data, including technology acceptance model, playfulness, mood, presence and willingness to use, are also measured in order to construct bio-psycho model. We will study correlations between each dimension of performance or physiological data and psychological data. Also, we will further investigate how bio-psycho model can quantitatively affect the improvement of motor skills from perspective of rehabilitation. 研究期間:10008 ~ 10107
