    Title: 五十肩遠距復健系統之復健成效探討;Investigation of the Effect of Rehabilitation with the Tele-Rehabilitation System for Frozen Shoulder

    Abstract: 五十肩是肩部軟組織及關節囊腔受損的通稱,在臨床上是一種相當常見的疾病。患病者會有肩關節疼痛、手臂無法舉高或轉動手臂等症狀,嚴重者將影響到患者的日常生活動作、睡眠等。而治療五十肩必須以肩關節復健運動搭配物理治療方能逐漸康復,但復健治療過程既辛苦且漫長,傳統復健方法的無趣、來回奔波住家與醫院間的辛勞,常使患者半途而廢,不願繼續治療。因此我們開發了一套結合虛擬實境、穿戴式裝置與遠距復健技術的五十肩遠距復健系統,招募罹患五十肩的病患前來參與使用,並設計實驗流程驗證本系統之有效性。
    本研究將受試者分為實驗組:使用五十肩遠距復健系統,並自行操作使用系統進行復健運動者,控制組:僅使用本系統中的五十肩復健系統部份,並由物理治療師全程輔助操作系統進行復健運動者,並比較此兩組受試者在臨床評估指標與系統紀錄復健表現的前、後測與期間進步幅度之差異性。
    實驗結果顯示:實驗組與控制組之間在上述比較中,大多為無顯著差異,也就是說,實驗組在沒有治療師幫助使用系統的情形下,依然能夠達到與控制組一樣好的復健成效。而我們也設計了問卷來調查實驗組之受試者對此系統的觀感,統計結果指出,受試者對於系統的有用性及社會臨場感皆持正面肯定,未來也願意持續使用本系統做復健。
    ;Frozen shoulder, medically referred to as the soft tissue of shoulder and articular capsule injuries. It is a common disease in clinical practice. The Symptoms of the patients who suffer from the shoulder problem were pain at the shoulder joint and having difficulty in raising arms, and it would seriously disturb patients′ daily life and sleep. The treatment methods for the disease were exercise for shoulder joint rehabilitation going with physical therapies and patients could increasingly recover from the disease. With these reasons: the long and hard rehabilitation treatment, the boring traditional method of recovery, and the faraway distance between home and hospital, patients often quit their rehabilitation. So we develop a system which combine virtual reality, wearable technology and tele-rehabilitation technique. And we recruit some subjects who suffered from Frozen Shoulder to join our research which mainly aimed to prove the effectiveness of this system.
    Subjects were divided randomly into experimental group and control group. Experimental group did rehabilitation exercise with the Tele-Rehabilitation System for Frozen Shoulder and operated the system by themselves. Control group did it with the system which only contained the Frozen Shoulder Rehabilitation part with the help of physiotherapist. We will compare the differences of pre-test, post-test and the degree of progress between these two groups in their clinical evaluation indexes and the performance which recorded by the system. Results show that experimental group has no significantly differences with control group in these comparison. In other words, Experimental group, without physiotherapist′s help, has got the same good performance with control group in this research. Additionally we design a questionnaire for experimental group to understand their perception for this system. It mentioned that subjects thought the system useful for their rehabilitation and gave the positive affirmation in social presence and has willing to continuously use it in the result.
