Abstract:	B、C肝炎可說是我國的「國病」，因其難以追蹤、症狀不明顯、難以治療與追蹤時間與間隔皆長的疾病特性，使得許多慢性肝炎患者常常失去警覺，甚至忽視醫師的諄諄告誡，當感到不舒服時，往往已經錯過治療的黃金時機，造成無可彌補的遺憾。罹患慢性肝炎者，除了必須終身追蹤複檢或治療，更需要病患及家屬的共同合作，協助患者做好健康管理，因此唯有透過良好的個案管理機制，才能增加肝炎的療癒成效。本研究透過與醫療人員的專業訪談，將其目前在肝炎個案管理上所面臨的困境與以歸納分析後，定義出一個良好的個案管理系統應具備的條件與能力，並實作一肝炎個案管理系統，來減輕醫療人員的業務負擔、增進個案管理成效。本研究實作之個案管理系統經醫師實地使用後，以深度訪談方式獲得以下結果： 1. 在使用介面及操作流程設計方面，本系統確實改善了現有醫院資訊系統的不足。 2. 在個案追蹤管理方面，本系統對於個案用藥、診療及檢驗結果呈現方式的設計理念，具體幫助醫師在短時間內，了解個案完整的治療歷程與數值變異情形，並做出診斷。 3. 本系統幫助醫師獲得現行治療方式對於個案的效度參考，具體幫助了治療照護計畫的擬定。 4. 在個案管理的做法及流程上，本系統大量減少了原本需要人工作業的事務，即使醫院沒有個管師的設置，醫師也能更輕鬆的管理個案。 5. 本研究改善了現有衛教單的呈現方式，使更貼近個案狀況，增加個案對衛教的遵從程度。 6. 本研究採用Web平台實做系統，架設容易，且可免除佈署與版本更新的負擔，也不會影響現有資訊系統之運作。 Hepatitis B and C is a common disease in Taiwan. A lot of patients die from it because they don’t feel the symptoms much, and the symptoms are not easy to be discovered, traced and cured. It is usually too late when the patients feel the pain. Chronic hepatitis patients not only have to be traced or cured in their whole lifetime, but also need to cooperate with families to manage their health. Therefore, a well management is the only way to help cure the hepatitis. In this study, we interviewed the medical personnel about the difficulties of the case management of hepatitis treatment that they are facing now, and then we analyzed and defined the conditions and abilities that a good management system should be provided with. After that, we implement a hepatitis case management system to lighten the medical personnel’s burden and promote the effect. The case management system in this study has applied by the doctors, and the results are as follows: 1. The user interface and the design of operation in this system have improved the data system in the hospital indeed. 2. About the management of project trace, the way that this system shows the results of medication, treatment, and exam specifically helps the doctor to see the complete course of treatment and the changes in a short time, and diagnose according to the results. 3. The system helps the doctor to refer to the treatment effect and specifically set up a course of treatment. 4. About the operation and the process of the management system, it massively reduces the work of the staff. The doctor can easily manage the case without the case manager. 5. The study improves the display of the handout forms of patient instructions, which is more suitable for each patient, so that the patients are more willing to follow the directions. 6. The system is implemented in Web-based platform. It’s easy to set up, and dispense from the loading of version updating and distribution. Moreover, it doesn’t interfere with the operation of the original information system.
